Tournament Forms and Paperwork required at Tournament Registration:

All teams must have:

• A signed Medical Release Form for each team member
• A Media Release Form for each team member

In addition:

Challenge A, B, C, E:

• 5 Copies of the 2 -page Tournament Data Forms
• 2 Copies of the Declaration of Indepence
• 1 Copy of the Expense Report

Challenge D:

• 5 Copies of the 1-page Tournament Data Form
• 2 Copies of the Declaration of Independence

Rising Star: Weighty News:

• 4 copies of the Starry Showcase Form
• 1 copy of Declaration of Independence (Optional)
• 1 copy of Expense Report (Optional)

Project Outreach:

• Forms required for Album - Planning Process, Project Outline, Project Activities, Community Partners, Financial Resources, Proeject Results & Bibliography
• 5 copies of 1-page Tournament Data Form
• 3 Copies of the Declaration of Independence
• 1 Copy of Expense Report

DI eXtreme

• No additional Forms Required
